How to Prepare for your visit
Welcome to First State Cardiovascular! To ensure your visit is a pleasant experience, we kindly request a few things in advance.
When you speak with our appointment scheduler, they will guide you through providing the necessary information. This includes details like your personal demographics, primary care physician, insurance information, and reason for your appointment.
To assist your cardiologist in assessing your condition or concerns effectively, we would be glad to obtain any relevant documents from your medical history. This may include items such as echocardiograms, EKGs, or cardiac catheterization records, whether they are from a past hospital stay, an emergency visit, or your primary care physician. To proceed, please complete the Release of Medical Records form and send it to our office before your scheduled appointment.
If your medical insurance requires a referral, kindly reach out to your primary care physician and inform them of your scheduled appointment. They will need to initiate the referral process. For many insurances, our NPI number (National Provider Identifier) is required. Our NPI number is 1912606401.
Lastly, please ensure you arrive 15 minutes prior to your new patient appointment.
